In order to receive your laptop, you MUST participate in a laptop orientation.
This laptop orientation registration form is only for those students that are mandated into the laptop program or who have opted into the laptop program.
Beginning April 29, 2008, students that wish to order a laptop through UIW will need to sign a letter of intent at the Business Office. The Business Office will then charge your account for the current/upcoming semester. The letter of intent can be signed in person or signed then faxed to the Business Office. If you are a minor, we will need your parent/guardian to also sign the letter of intent plus a copy of your parent's/guardian's ID. To sign in person, stop by the Business Office located in the Administration bldg. Simply download, print, fill out, and fax the signed letter of intent plus a photo id (for verification of identity) to 829-2745. To ensure the laptop's arrival before the semester begins make sure the signed letter of intent is turned in to the Business Office by the end of business on July 31, 2008.
Students please arrive at least 15 minutes prior to your scheduled Orientation class. Anyone arriving late after Orientation class has begun will have to reschedule for another class.
